IMPACT (IC3)
IMPACT, also known as the Intensive Customized Care Coordination (IC3) Program, is part of Aspire Youth and Young Adult Services. It supports youth ages 5 to 20 and their families who are facing serious behavioral health challenges at home, in school, or in the community.
IMPACT uses the High-Fidelity Wraparound model. This team-based approach brings families, providers, schools, and community supports together to create a care plan based on the strengths and needs of each youth and family.
Rather than depending on one service, IMPACT helps coordinate care across several systems. This helps families understand available services, build stronger support systems, and create long-term stability.
How IMPACT Works
Each youth and family works with a Care Coordinator who helps organize services and connect them with resources. The Care Coordinator also helps make sure everyone involved is working toward the same goals.
Services are provided in the community and in settings where families feel most comfortable.
The Wraparound process includes:
- Care planning based on the strengths and needs of the youth and family
- Coordination with schools, providers, and community supports
- Monthly Child and Family Team meetings to review progress and shared responsibilities
- Ongoing review and changes to goals and supports when needed
- In-person contact with families at least once each week
IMPACT does not provide therapy directly. Instead, the team helps coordinate clinical and community services based on each youth’s needs.
Who the Program Serves
IMPACT supports youth ages 5 to 20 who have emotional, behavioral, or other challenges that affect daily life.
The program may support youth experiencing:
- Anxiety or depression
- Difficulty managing emotions or behavior
- Attention and concentration challenges
- Trauma or difficult childhood experiences
- Substance use concerns
- Family or school-related challenges
- Difficulty staying stable at home, in school, or in the community
IMPACT may also help youth and families who need more coordination among providers, schools, and community agencies.

Success in IMPACT may include better daily functioning, stronger family stability, fewer crisis events, and greater success at home, in school, and in the community.
By surrounding families with coordinated support and including them in decisions, IMPACT helps create a clearer path forward.
