Permanent Supportive Housing Rural Program
The Permanent Supportive Housing Rural Program provides long-term housing support and coordinated services for individuals and families experiencing homelessness and living with disabilities. The program follows the same model as Aspire’s Permanent Supportive Housing Program and focuses on serving individuals in the western counties.
The program helps individuals find and maintain safe, stable housing. It also connects them with services that support health, recovery, and greater independence.
Program Services May Include:
- Long-term rental assistance
- Case management and care coordination
- Connections to mental health and substance use treatment
- Connections to medical, social, and community services
- Support with life skills and greater self-sufficiency
- Coordination with community partners and housing resources
Who the Program Serves
The program serves eligible individuals and families experiencing homelessness who need ongoing support to maintain stable housing. Individuals may also be living with disabilities, chronic health conditions, serious mental illness, or substance use challenges.
Eligibility is based on program requirements, funding guidelines, the service area, and individual or household need.

Success in Rapid Re-Housing
Success may include obtaining permanent housing, maintaining housing stability, increasing income or employment when possible, and improving financial stability. The program also helps individuals and families build stronger connections to community resources and support.
