Intensive Community-Based Services (ACT & CST)

Aspire’s Intensive Community-Based Services help individuals build the skills and support needed to remain stable and avoid unnecessary hospital stays. Services are provided in the community and are based on each individual’s needs.

Assertive Community Treatment (ACT)

Assertive Community Treatment (ACT) is an intensive, community-based program for individuals living with severe and ongoing mental illness. Often called a “hospital without walls,” ACT provides mental health services directly in the community.

ACT offers support 24 hours a day, 7 days a week. The program meets individuals where they are to help them manage their health, maintain stability, and reduce the need for hospitalization.

Individuals receive frequent support from a team of professionals. The team includes clinical staff and Certified Peer Specialists who provide recovery support based on lived experience.

Services include:

  • Regular in-person visits from a dedicated treatment team
  • Medication management and nursing support
  • Individual counseling and therapy
  • Help accessing housing, employment, and social services
  • Crisis intervention and ongoing recovery support

Success in ACT may include greater stability, better follow-through with treatment, and fewer hospital stays. By providing support in the community, ACT helps individuals live safely and as independently as possible.

Community Support Team (CST)

The Community Support Team (CST) provides intensive mental health support for individuals moving between different levels of care. The program is designed for rural communities and brings services directly to individuals in their homes, workplaces, and communities.

CST helps fill the gap between outpatient care and higher levels of treatment. The team provides ongoing support during times of change or when an individual needs more help staying stable.

Services include:

  • Therapy and counseling in the community
  • Nursing support and medication management
  • Case management and coordination with healthcare providers
  • Help connecting with housing, benefits, and local resources
  • Crisis prevention and early support

Success in CST may include greater stability, stronger community connections, and less need for hospital or crisis services. The program helps individuals build the skills and support needed to live healthier and more independent lives.

CST services are available to residents of Calhoun, Early, Miller, Seminole, and Decatur counties.
