Evolve Program
The Evolve Program supports young adults ages 16 to 30 who have experienced a first episode of psychosis within the past two years. Early support is important, and Evolve provides coordinated care to help individuals regain stability and move forward with confidence.
Each individual works with a team of professionals who support mental health, education, employment, and personal goals. Team members work together to make sure services are connected and based on the individual’s needs.
Program Services Include:
- Individual therapy and psychiatric care
- Case management and community support
- Supported employment and education services
- Peer support from individuals with lived experience
- Family education and involvement in recovery planning
Success in Evolve may include greater stability and independence. Individuals may return to school, find employment, maintain stable housing, and build the confidence needed to manage their mental health over time.
